Health cadres play a strategic role as a bridge between communities and primary health care facilities, particularly in promotive and preventive activities. Limited cadre involvement may contribute to suboptimal health service quality, including low Posyandu coverage, delayed detection of health problems, and limited community participation. This study aimed to analyze the relationship between health cadre involvement and public health service quality based on patient perceptions in the working area of Ambacang Padang Health Center. A quantitative cross-sectional design was employed involving patients who received health services at the health center. A total of 96 respondents were selected using accidental sampling based on predetermined inclusion criteria. Data were collected using validated and reliable structured questionnaires assessing cadre involvement and perceived health service quality. Data were analyzed using univariate analysis and the Chi-square test at α=0.05. Results showed that 54.2% of patients perceived cadre involvement as low, while 57.3% perceived health service quality as poor. A significant relationship was found between cadre involvement and health service quality (p=0.000; OR=3.98; 95% CI: 1.76–8.99). Strengthening cadre training, supervision, and performance monitoring is recommended to enhance primary health service quality and community participation.
